What a GLP-1 program requires clinically and legally
GLP-1 weight-loss telehealth has changed more in the last 18 months than in the five years before it. The requirements below reflect our understanding as of September 2026. They are general information, not legal or medical advice, and your own counsel and medical director should confirm how they apply to your program.
FDA-approved products come first
Wegovy (semaglutide) and Zepbound (tirzepatide) are FDA-approved for chronic weight management and are dispensed by retail and specialty pharmacies on a standard prescription. As of September 2026 they are the default route for a compliant GLP-1 program. Manufacturer self-pay programs have made cash-pay access more predictable, though pricing changes often and brands should confirm current terms rather than quote a number.
Compounded semaglutide and tirzepatide are limited, not banned
FDA removed tirzepatide from the shortage list in December 2024 and semaglutide in February 2025, and the compounder grace periods ended in spring 2025. Since then, compounding a product that is essentially a copy of an approved drug is not permitted for 503A or 503B facilities, and in April 2026 FDA proposed excluding these drugs from the 503B bulks list. What remains, as of September 2026 and subject to change, is patient-specific 503A compounding where the prescriber documents a need the approved product does not meet, such as an excipient allergy or a dose not commercially available. That call belongs to the provider. A program built on volume compounding of copies is not one we will power.
State telehealth and prescribing rules
Each state decides whether an initial GLP-1 prescription can follow an asynchronous intake or requires a synchronous video visit, and several have tightened rules for injectable weight-loss drugs. Providers are licensed in the patient's state and OrbitOS applies the modality the state requires. Rules move often, so treat any modality map as a snapshot.
Corporate practice of medicine and the MSO structure
Most states restrict non-physicians from owning a practice or directing clinical decisions. The standard answer is a management services organization: your brand handles marketing, technology and administration while a separately owned professional entity employs the providers. MyOrbitHealth operates on this structure so you can launch without forming a PC in each state. Your counsel should confirm how it applies to you.
Baseline labs and monitoring
GLP-1 protocols commonly include a metabolic baseline before starting and periodic monitoring after. Typical panels include a comprehensive metabolic panel, lipid panel, HbA1c, thyroid function and, where indicated, lipase or pregnancy screening. Whether labs are required at intake, ordered on a risk basis or deferred is a clinical policy the medical director sets, not a marketing choice.
Advertising and LegitScript certification
As of September 2026, Google requires LegitScript Healthcare certification before ads referencing semaglutide, tirzepatide, Wegovy, Zepbound or compounded GLP-1s will run, and Meta, Microsoft and TikTok have comparable gates. LegitScript reviews provider structure, pharmacy partners, claims and privacy posture, and has said it will not certify programs built on compounded copies. Certification is decided by LegitScript, not by any vendor.

For Wegovy and Zepbound, that is a retail or specialty pharmacy. Where the provider documents a patient-specific need, the prescription can route to a LegitScript-certified 503A compounding pharmacy in the network. Refrigerated products ship cold chain with status visible in the portal and app, and we take 0% markup on medication.
Orbit Labs for metabolic baselines and monitoring
When the medical director's protocol calls for labs, the provider orders the panel through Orbit Labs and the patient chooses how to collect: a walk-in order slip at any Quest Diagnostics or Labcorp patient service center, a Tasso at-home kit shipped to the door and returned by FedEx, or a mobile phlebotomy nurse who draws at home or work. Results return to the ordering provider, who reads them into the plan before prescribing or titrating. Your storefront can also carry a patient self-order lab catalog, with every order signed by a provider.
Branded storefront, app and subscription billing
Patients buy on your domain through a storefront with checkout and recurring subscriptions, and use a native iOS and Android app for messaging, check-ins, weight logging and refill status. You are merchant of record and patient data belongs to you. Subscription cadence, bundling and pause or cancel flows are configured at onboarding.
Follow-up cadence and titration in OrbitOS
GLP-1 therapy is a titration schedule. OrbitOS tracks current dose, next step-up, side-effect reports from the app and the date the fill runs out, then queues the follow-up and renewal before the gap. The medical director sets the cadence, often a check-in at each dose change and monthly review during titration, then longer intervals on maintenance.
Managed LegitScript certification and compliance posture
We prepare, file and manage your LegitScript application through approval. Once filed, approvals typically come back in days, though LegitScript decides and it is not guaranteed. - Only in a narrow way. As of September 2026, semaglutide and tirzepatide are off FDA's shortage list, and compounding a copy of an approved product is not permitted. A provider may still prescribe a patient-specific compounded product from a 503A pharmacy when there is a documented clinical need the approved drug does not meet. That decision is the provider's, case by case. We do not power programs that depend on volume compounding.
- How much does compounded semaglutide cost through a program on MyOrbitHealth?
- We do not quote medication prices, and we take 0% markup, so what the patient pays is the pharmacy's price passed through your storefront. Compounded pricing, where it is clinically appropriate, is set by the dispensing pharmacy and varies by dose and formulation. Wegovy and Zepbound pricing has changed several times since 2025, so check current self-pay and coverage terms rather than any published figure.
- Do patients need labs before starting a GLP-1?
